Job Information

Are you passionate about helping others to live a happy and fulfilled life?
Are you confident, self-motivated, flexible, and have a ‘can do’ attitude?

This is an exciting opportunity to join a new, flagship Department of Health funded project working with older people and adults with multiple needs. Imago is a leading charity providing support and opportunities to people across the South East. You will be responsible for a dedicated area, and following training, will independently manage a caseload and new referrals.

Referrals are received from hospitals and GP’s. You will meet with clients in medical settings or in their own homes to carry out holistic assessments; and together design plans with the aim of reducing hospital readmissions and improving health and wellbeing. The service offers short-term intensive support; building trusted relationships to improve confidence and motivation, encourage personal choices and to support others to access a range of community, health and social care options.

You will respond to challenging and complex situations with a sensitive and measured response, in line with Imago Safeguarding and Lone Working Policies.

In this role you will bridge the gap between primary care professionals and social opportunities, so a knowledge of local services and the ability to build effective networks with cross sector organisations is essential.

Post is subject to references and an enhanced Disclosure and Barring Service check. A full, clean driving licence and ownership of a vehicle is essential as you will be working in different locations.

You can make a difference and be part of the solution.

To apply, please either submit your CV with a short covering note outlining your CURRENT SALARY or visit our website at to download a full Job Description and application pack.

Imago is a long established, well respected and effective charity working across Kent, Medway, East Sussex and London delivering a range of services to individuals, voluntary and community organisations, the public and private sector.
Our mission is to promote, develop and facilitate social action, support independence and empower communities. User and volunteer engagement, leadership and involvement in decision making are core principles.

We deliver a wide range of services in partnership with funders, public sector commissioners and private sector sponsors to children and young people, older people, carers, and volunteers.

Imago offers a competitive remuneration package consisting of salary, contributory pension, healthcare benefits and 25 days annual leave plus Bank Holidays.

Underpinning our work is a commitment to excellence, evidencing impact and safeguarding vulnerable people.

Imago recognises that many people in our society experience discrimination or lack of opportunity for reasons that are not fair. These include: race, religion, creed, colour, national and ethnic origin, political beliefs, gender, sexual orientation, age, disability (including mental illness), HIV status, marital or civil partnership status, pregnancy or maternity/paternity status, responsibility for dependants, appearance, geographical area, social class, income level or criminal record.
Imago aims to create a culture that respects and values each other’s differences. Imago sees these differences as an asset to our work as they improve our ability to meet the needs of the organisations and people we serve. We proactively seek to increase opportunities for inclusion and celebrate diversity across our organisation and within the communities we serve.
